Notes for IRS 4506-C form:
We must have the entire IRS 4506-C form hand written or typed – not mixed. All names, addresses, and social security numbers MUST match the MOST RECENT tax returns. Lines 6 through 9 must match requested order. Make sure there are no visible corrections to the form. If a mistake is made, you must start over with a new form.
